2018 Fountain Award Entry

Please complete the required fields and submit your online entry. You will need to complete one online form for every entry.

You will receive an email reflecting all of your information upon completing the form.

You will need two copies of the submission form/email with each entry. One copy MUST be attached to the back of your entry and one copy MUST be included with your check or copy of online payment confirmation. Entries are due no later than Friday, 4/6/2018. Hours accepted are M-F 9:00 am to 4:30 pm at EAG - 2029 Wyandotte, Suite 101, Kansas City, MO 64108.

Each entry will be judged on the following criteria:

Creative - 25%
Production - 25%
Objective/Strategy - 25%
Results - 25

4. Objective and Strategy - This will count towards 25% of your score. *This question is required.
